Meet Cindy
Cindy has 10 years of schooling with an ADN and BSN through WSU; her MSN through the University of Phoenix; and a Post Master's degree in FNP through Indiana State University. Cindy has a plethora of experience, which includes: 15 years of nursing experience in several local hospitals working in many different units and teaching experience at WSU Nursing for 9 years. In addition to owning her family practice, she works with Applegate Hospice, AOS podiatry, Solset Wellness (in SLC), Happy Valley Regeneration. A big part of her practice is doing HouseCalls (Assisted living facilities/Skilled nursing facilities and homebound patients for the elderly and disabled population). Cindy has vast experience in family medicine for the last 16 years as a provider! She considers herself a jack of all trades and loves that she can do so many things for her patients. She has loved venturing into functional medicine, as she feels this is very important in each person's health- to heal the body in healthier ways besides pharmaceuticals. She has also had several years diving into regenerative medicine by using Wharton's jelly and Adipose Derived Allograft stem cells. She has seen amazing things that this has done for her patients.

My favorite patients are the most difficult; the patients where other providers have given up on them; the patients who were told there's nothing more that can be done or it's just because you're depressed without digging deep; where patient's never felt heard before, I will listen and act. Meet Amy Jones
With eight years of dedicated experience in healthcare, Amy is a compassionate and skilled Family Nurse Practitioner. Her extensive background spans various healthcare settings, including hospitals, nursing homes, rehabilitation centers, and home care, where she has been committed to enhancing patient outcomes and delivering high-quality care.

Amy has a proven track record in conducting thorough health assessments, managing complex conditions, and working collaboratively with multidisciplinary teams to develop and implement effective treatment plans. Her experience includes addressing a broad range of healthcare needs and providing comprehensive care for both chronic and acute conditions.
Amy’s commitment to patient-centered care is evident in her ability to educate patients and their families, advocate for their needs, and facilitate effective communication with healthcare providers. Her strong analytical skills, combined with an empathetic approach, enable her to tailor care plans to meet each patient’s unique needs.

Meet Heidi LaMarr
Heidi is a dedicated healthcare professional who has devoted the majority of her adult life to improving the well-being of others. Her journey in the medical field began as a Certified Nursing Assistant (CNA), where she spent many fulfilling years in home health and hospice care. Currently, Heidi serves as the office manager for Duke Family Wellness, where her commitment to patient care continues to shine through in every aspect of her work. Her extensive medical background is complemented by a deep compassion for others, making her an invaluable asset to both her colleagues and the patients she serves.

Receptionist / / CNA
Gabi Duncan helps manage the business phone, schedules patients, makes sure you have what you need before your visit, and will be helping the FNPs at your visit! ​She is taking pre-requisite nursing classes full-time and hopes to enroll in the nursing program next year. She just received her CNA. She loves taking care of others and volunteers with Applegate Hospice.
